Tetsu Nakamura, 73, a representative of the Peshawar-kai aid group based in the southwestern Japan city of Fukuoka, was injured and his bodyguard, driver and three other people were killed, the official said.
The region is a hotbed of Islamic extremism.
Anti-government Taliban forces have been active there for some time, and a branch of the Islamic State militant group has been active in the region recently.
